Find out without blame – Root Cause Analysis
Does your team go into blame mode when something goes wrong?

Recently manufacturers joined to learn and discuss how to find the root cause of a problem without the blame. The presentation by Chad Metcalf, Value Stream Solutions Inc., included information on Quality Controls vs Quality Assurance and the tools needed to find Root Cause.
Quality Control measures are a temporary method to fix the problem, however adding additional check points into your process without changing or amending the process allows the problems to still occur. While Quality Assurance is a permanent solution where you build in Quality at the source to eliminate the problem. To do this, you first need to find your root cause.
How do we find root cause and what tools are used?
Two problem solving tools discussed were the 5 Why method and the Fishbone method and when to use which method to find your root cause.
Lastly, how do you know when you have reached the Root Cause? Chad provided the group with the final questions to ask yourself to confirm you have found the source of your problem.
